Congratulations! You've come to the right place.

Selecting the right person to make decisions on your behalf is crucial. So in order to make this process as easy as possible, we’ve developed four forms.

Simply select the relevant form category, enter the state of Australia you live in and make payment. You’ll then be able to download the form as a PDF file that can be edited on your computer or, if you prefer, can be printed and filled out by hand.

Please note: our sample forms are just that, a sample, and are only to be used as guide to show you the indicative format your form will come in.
